What to expect, step by step.

Your first visit starts with a conversation about what you want. Records and photos come next, then a clear review of what we found and your options.

  1. A scheduling call. We learn what you need and answer your questions.
  2. A welcome package. Sent before your visit, so you know what to expect.
  3. Arrival. We greet you by name, and you meet your treatment coordinator.
  4. Your goals, then records. A conversation about your goals and concerns, then records and photographs.
  5. Your options, explained. We review your photos and findings together and walk through each option.

From scan to smile, under one roof.

Your implant case is scanned, planned, placed, and restored in our office, with the same doctor following it from the first scan onward.

  • Scan: A 3D CBCT scan, digital scans of your teeth and bite, and a full-head facial scan.
  • Plan in 3D: Implant position is planned against your bone, nerves, and sinuses before surgery, and new teeth are planned to fit your face.
  • Place: Dr. Patel places implants in our office, with same-day provisional teeth when clinically appropriate.
  • Restore: Crowns and bridges are designed from your scans; long-term temporaries are 3D-printed in-house, and most final crowns are made by a dental lab in ceramic or zirconia.

Shivam S. Patel, DDS

Dr. Patel earned his DDS from the University of Illinois Chicago College of Dentistry in 2012 and has completed more than 1,500 hours of implant-focused continuing education since. He is an Associate Fellow of the American Academy of Implant Dentistry and founded the Midwest Institute for Advanced Education, which trains general dentists in implant dentistry.

We’re at 545 S York Rd, Suite 200, on the east side of York Road between Crest Avenue and Red Oak Street, just south of Jefferson Street and across York Road from White Pines Golf Club.

An adjoining lot for patients and staff, plus free street parking directly in front of our entrance.

Our office is on the second floor, up a short flight of about 8–10 steps. There is no elevator, and the office is not wheelchair accessible at this time. If stairs are difficult for you, please call us before your visit so we can talk about your needs.

Our phone line is answered 24/7. After hours, tell our automated receptionist it’s an emergency and your call is forwarded to Dr. Patel.

Is Dr. Patel a general dentist or a specialist?

Dr. Shivam S. Patel is licensed in Illinois as a general dentist. He has completed more than 1,500 hours of implant-focused continuing education since 2012 and is an Associate Fellow of the American Academy of Implant Dentistry (AFAAID).

Does Dr. Patel place and restore implants himself?

Yes. Dr. Patel plans, places, and restores every implant case in our office, and he delivers final teeth himself.

What insurance do you accept?

MCDI is in-network with Delta Dental Premier. We accept most PPO plans with out-of-network benefits and file claims for you. Financing is available through Cherry, Proceed, and CareCredit.

What are your hours?

Monday 9–5, Tuesday 9–6, and Thursday 9–5. Wednesday and Friday are reserved for scheduled surgery and complex cases. Closed Saturday and Sunday.

Do you have Polish-speaking staff?

Yes. A Polish-speaking member of our clinical team can help. Mention it when you schedule, and we’ll plan your visit around it.

Is the office wheelchair accessible?
