Referring a patient to MCDI
Midwest Center for Dentistry and Implants (MCDI) accepts referrals from dentists for implant and full-arch treatment, complex restorative cases, extractions, bone grafting, and snoring therapy. Dr. Shivam S. Patel is a general dentist licensed in Illinois and an Associate Fellow of the American Academy of Implant Dentistry. When you’d like to keep the restorative phase, we return your patient to you for it.
What we see referrals for
- Implant evaluations, planned from a 3D CBCT scan taken in our office.
- Single-tooth implants and implant bridges.
- Full-arch treatment, fixed or removable (implant overdentures).
- Extractions and socket preservation.
- Complex restorative cases: full-mouth restorations, worn teeth and a collapsed bite, and plans that combine implants with crowns or bridges on natural teeth. We also repair or redo failing crowns, bridges, and implants.
- Bone grafting and sinus lifts.
- Wisdom teeth: some are removed in our office; complex cases close to the nerve are referred to an oral surgeon, and we’ll tell you if that’s the case.
- Snoring: after a home sleep test and diagnosis by a physician through Sleepwell, we provide oral appliance therapy, QuietNite Therapy (a three-session CO₂ laser protocol), or both.
How a referral works
- Send the referral. Fax the referral form to (630) 766-1164 or call (630) 766-0115. Send radiographs if you have them; if not, we take a CBCT and any other images we need here.
- We see your patient. The first visit is a consultation with Dr. Patel and a 3D CBCT scan.
- We report back within a week. We fax or email our findings to your office within a week of the consultation, and we share 2D and 3D radiographs with you.
- You choose the restorative phase. Mark on the form whether the patient returns to you for the final restoration or Dr. Patel manages both phases.
